What is reason causeing linpack failed?

I have had this happen when I installed bad memory 
into the machine.  Actually, it was either bad memory
or a bad simm slot on the motherboard, but it did cause
the job to fail.  Removing the memory fixed the problem.

There could be other reasons as well.
